On 3 November 2019, the pilot of the ASW-19 glider, OK-4481 registration mark, took off in aerotow from the Mikulovice airfield to mountain wave soaring behind the range of the Jeseníky mountains. For about 2 hours, the pilot was gradually climbing up to an altitude of 6,759 m MSL. Subsequently, after about 9 minutes of flight at an altitude above 6,600 m MSL, about 0.5 km west of the village of Ludvíkov, the glider went into a manoeuvre ending in a sharp loss of altitude. During the manoeuvre, the wing structure failed. The fuselage and tailplanes were destroyed due to the impact on the ground.

At the end of the flight day on 3 November 2019, none of the LKMI operators discovered that the pilot of the glider ASW-19, registration mark OK-4481, had not landed at LKMI. Only on the following day, 4 November 2019 at 08:54, RCC Prague received information from LKMI about the search for the glider. A DETRESFA was declared, and military and police helicopters designated for search and rescue operations were summoned. At 11:09 a.m., the site of the glider’s fuselage impact to the ground, and also the pilot’s body with no signs of life, was found. The Police of the Czech Republic, the FRS, and the AAII inspectors with a forensic doctor arrived on site of the accident on 4 November 2019 and the AAII inspectors conducted investigation.

The cause of the air accident was a loss of control of the glider which resulted in the glider going into a descending, steep right turn. The reason for the loss of control was not precisely determined, but the possibility that the pilot became incapacitated due to symptoms of hypoxia or fatigue could not be ruled out.
